Request for Leave of Absence – FMLA CFRA PDL – English » Spanish » Provide this form if you’re an employer covered by the federal Family and Medical Leave Act (FMLA) or the California Family Rights Act (CFRA) and either an employee has requested a leave of absence or you recognize the need.

What is a medical certification letter?

In short, a medical certification is a relatively short form that is filled out by a health care provider and provided to the employer to establish a patient or family member’s medical condition that requires FMLA-protected leave.

What is a wh 380 E?

Form WH 380-E, Certification of Health Care Provider for Employee’s Serious Health Condition, is a form used by employers and sent to the US Department of Labor, Wages and Hour Division. This form verifies that an employee has a serious medical condition.

How do you complete WH-380-F?

Fill out Section 2 of the form.

If you are completing form WH-380-F, you will be required to provide information about the family member you are caring for during FMLA leave
